A Care Manager acts as a liason, mediator and/or a neutral third party with family members while coordinating the use of all applicable benefits and entitled services. The search for various services often requires the knowlege and efforts of a qualified Care Manager. Today, people are living longer and wish to remain in the comfort of their own home. Families of individuals who have had a significant change in physical and/or mental status are faced with many challenges. Overwhelming options can be especially difficult while family members are engaged in their professions, commitments to minor children, personal endeavors, or quite often live out of the sunshine state, Florida. Care Management allows families to select services that compliment and serve their specific needs to alleviate stress.
A comprehensive assessment and implementation of a Care Plan is the core of the care management process. The assesment takes place wherever the client calls "home". From the initial meeting, I encourage family participation while gathering pertinent details such as the client's medical history, social desires and living environment. Essentially, a Care Manager acts as your liasion, your eyes and ears. Ongoing communication is vital to alerting family members as to changes in health and mental status. My number one goal is to provide a sense of calm, a peace of mind to families regarding the multi faceted needs of their loved one during the aging journey.
